How do I confirm an appointment?
When supported by your provider, you can confirm your appointment in the app. Confirming an appointment notifies your provider that you will arrive at the provider’s office at the scheduled location, date, and time. Notes: Appointments can only be confirmed within 72 hours of the appointment date. If the appointment is not yet within the 72 hour window, the confirmation option will not be available to you. Confirm Appointment On the Homepage, locate the name of the provider and the appointment date. Click the Confirm Appointment link. In the confirmation window, click the Confirm Appointment button. View Confirmation Once the confirmation is complete, the appointment confirmation item displays a solid green circle with a white check mark [1]. Depending on your provider, you may have additional check-in tasks to complete [2].

How do I update my medical history for an appointment?
Once your appointment is confirmed, you may be asked to review and update a medical history questionnaire or form in the app. If you have previously completed an initial medical history for an initial appointment, subsequent appointments may display a pre-filled medical history. If your medical form supports pre-filled data, you can view the last date the history was updated, and you can update any medical history items as needed. If your medical history form is blank, learn how to fill out the medical history form. Notes: The medical history should be completed within 1 hour of your appointment date. You can leave the app and resume your medical history update at any time. Open Medical History In the Homepage, locate the name of the provider and the appointment date. Click the link for the form or questionnaire. Note: Depending on your provider, the name of the form or questionnaire may vary. Update History If you need to make changes to a category, click the name of the category. Make the appropriate changes in the category as needed. When you are finished completing information for the current category, click the Next button [1]. You’ll be taken to the next category in the questionnaire. Note: If you need to finish updates in your Medical History later, click the Save For Later link [2]. You can return to the Medical History link in for the appointment and finish the questionnaire at a later time. Finish History When you are finished with your medical history updates, click the Finish button. View Completion Once your medical history is complete, the Medical History confirmation item displays a solid green circle with a white check mark [1]. How do I fill out a blank medical history form for an appointment?
Once your appointment is confirmed, you may be asked to review and update a medical history questionnaire or form in the app. A questionnaire may ask various medical history questions, such as health conditions, prior procedures, current medications, lifestyle, family history, and other details. Notes: The medical history should be completed within 1 hour of your appointment date and time. You can leave the app and resume your medical history update at any time. If your medical history form has been pre-populated for you, learn how to update your medical history if needed. Open Medical History In the Homepage, locate the name of the provider and the appointment date. Click the link for the form or questionnaire. Note: Depending on your provider, the name of the form or questionnaire may vary. Start History The page displays all aspects of your medical history to complete for your provider. Note: If you have previously filled out your medical history, the app displays the date of your last medical history update, and all information is pre-filled in the questionnaire. Learn how to update your medical history. In the confirmation window, click the Start button. View Category The questionnaire will take you through multiple categories for your health history. For each category, review the included information. Any information already recorded by your provider will be displayed in the page. Add Item If you need to add a new item within a category, click the Add button. You can follow the prompts to add as many items as needed. Continue to Next Category When you are finished completing information for the current category, click the Next button [1]. You’ll be taken to the next category in the questionnaire. Note: If you need to finish the Medical History later, click the Save For Later link [2]. You can return to the Medical History link in for the appointment and finish the questionnaire at a later time. Finish History When all categories have been completed in the questionnaire, click the Finish button. View Completion Once your medical history is complete, the Medical History confirmation item displays a solid green circle with a white check mark [1]. How do I check in for an in-office appointment?
When supported by your provider, you can check in for an in-office appointment in the app. Notes: Appointments are available for check in one hour before the appointment time. The app check-in process will not be available after the time of your appointment has passed. If the app check-in option is not available to you, please contact your provider’s office upon arrival. Check In In the Homepage, locate the name of the provider and the appointment date. Click the Check In Appointment link. Finish Check In Review the check-in information as directed by your provider. When you are finished, click the Finish button [1]. Note: If you need to finish the Check In information later, click the Save For Later link [2]. You can return to the Check In Appointment link for the appointment and finish the information at a later time. View Completion View the confirmation page for your appointment.

How do I check in for a virtual appointment?
For virtual appointments, you do not need to officially check in via the app. Instead, you will log into the virtual link for your appointment. Notes: The virtual appointment link will not be available to you until you have completed all pre-appointment tasks. Access Appointment In the Homepage, locate the name of the provider and the appointment date. Click the link to access your virtual appointment. Depending on your healthcare provider’s configuration settings, you may be directed to your healthcare system’s login page to access the virtual appointment. However, if the link can be read by Tendo, the link will direct you to the virtual service.

How do I upload my government ID before an appointment?
Some healthcare organizations ask patients to upload their government-issued ID so that it can be on file and the organization can verify that a patient is who they claim to be. This is not mandatory, but it can save both you and your healthcare organization time during the pre-check-in process before appointments. If your healthcare organization offers this option, Upload government ID will be one of the pre-check-in tasks that will display in the Patient Care Journey app after you schedule an appointment in the app. The checklist will appear on the Appointment Details page or in Upcoming Appointments on the home screen, in the section that contains information about your appointment. If you don't see it on the home screen, click on View All to the right of Upcoming Appointments to see the list of your appointments and select the correct one. When you have completed the tasks that are above Upload Government ID on the checklist, check the circle next to the Upload Government ID task. This takes you to a modal on which if you have already uploaded your ID, you will see this view telling you that you have already uploaded it. If you haven't done so, or if you click Edit in the lower right corner to update your card, you will be taken to an Upload Government ID screen. There, you can upload front and back images of your government ID by dropping images of the front and back of your ID card in the appropriate places. Click Upload. You also skip this task by clicking Skip in the lower right corner. If so, a screen will appear advising you that you will need to present your ID in person at the appointment. After you upload your ID, it will persist in the system so that you won’t have to upload or present it for future appointments at this healthcare organization.
